Safe and Independent Living (SAIL)
SAIL is designed to provide person-centred, referrals and support to enable individuals to continue maintaining independence, improve their well-being and stay safe.
Who is the service for?
People over the age of 55, people over the age of 18 with a long-term health condition or disability and unpaid carers over the age of 18.
What does the service do?
The SAIL project is a quick and simple way to access a wide range of local services which support people in maintaining their independence, safety and wellbeing. The SAIL team can arrange one-to-one visits and complete a holistic, person centred assessment and together create an action plan. The team are able to offer short term support to enable the client to maintain independence. If they are not the best service to provide the support, then the team will identify services to make appropriate referrals to. They always follow up on referrals they make within four weeks to ensure the client has heard from the service and they’re receiving the support needed.
What happens after the referral?
Once they receive a referral form, the team will contact each individual.
